The lack of prior knowledge in mathematics is attributed to various factors that contribute to the high failure and dropout rates among first-year students in Chilean professional institutes. This research aimed to design and validate a diagnostic questionnaire to gather relevant information on variables associated with academic performance and educational inclusion, in order to support pedagogical decision-making. The questionnaire was reviewed by experts to ensure content validity and then administered to a sample of 69 students. Using R software, the data were analyzed to assess the reliability (Cronbach’s alpha) and factorial structure (exploratory factor analysis). The results support the instrument’s psychometric soundness and enable the identification of key factors affecting mathematical performance, providing a foundation for future research and educational improvements.
